Is the carbohydrate-insulin model the primary driver of obesity, or one factor among several?

Mainstream: Total energy balance remains the foundational principle — calories matter, even if hormonal signaling modifies how easy or hard adherence feels.

Contested: Elevated insulin from frequent and refined-carb-heavy eating is the primary driver of fat storage; lowering insulin enables fat oxidation more than calorie restriction alone (Bikman, Fung framing).

What survives: Structured eating windows and refined-carb reduction tend to make calorie balance feel more sustainable in practice. Both views can be true at once; matched-calorie trials at similar adherence show smaller differences than the strongest insulin-driver claims suggest.
